When you contact Elev8, book a consult, or become a member, we may collect your name, contact information, and — with your consent, once you're a client — health information relevant to your care, gathered during intake and ongoing treatment.

How we use it

To respond to consult requests and schedule appointments
To provide medical care, including coordination with labs, specialists, and other providers you authorize
To communicate with you about your membership and care
To meet legal and recordkeeping obligations required of a medical practice

How we protect it
